Learn more at phamily.com.</p> <p><strong>Job/Role Description</strong></p> <p>The <strong>In-Clinic Enrollment Specialist</strong> serves as the on-site representative of Phamily’s Advanced Primary Care Management (APCM) program within Medical Group practices.</p> <p>This role combines the <strong>clinical empathy and skill of a Licensed Clinical Social Worker (LCSW/MSW)</strong> with the <strong>operational purpose of patient engagement and enrollment</strong>.</p> <p>The specialist drives APCM enrollment in clinics, strengthens relationships between providers, patients, and care management, and ensures patients understand and benefit from ongoing care coordination services.</p> <p>This is a <strong>frontline, relationship-driven role</strong> essential to scaling Phamily’s partnerships and embedding care management as a core component of every patient’s healthcare journey.</p> <p><strong>Key Responsibilities:</strong></p> <ul> <li>Identify and engage eligible patients for APCM enrollment during in-office visits, leveraging Phamily’s platform and eligibility data.</li> <li>Explain the purpose and benefits of care management, emphasizing continuity of care and partnership with their providers.</li> <li>Serve as the <strong>in-office liaison</strong> between Phamily’s Care Management team and providers and clinical staff.</li> <li>Provide real-time updates to physicians and practice managers about patient enrollments, social needs identified, and care management follow-up activities.</li> <li>Conduct brief social needs screenings during patient encounters to identify barriers such as transportation, food insecurity, or housing instability.</li> <li>Provide resource referrals and warm handoffs to community organizations or Phamily care managers for complex cases.</li> <li>Track and report key metrics including number of patient interactions, enrollments completed, SDoH interventions, and provider engagement.</li> </ul> <p><strong>Requirements:</strong></p> <p><strong>Education &amp; Licensure:</strong></p> <ul> <li>Master’s Degree in Social Work (MSW) required.</li> <li>Active Licensed Clinical Social Worker (LCSW) or equivalent state licensure strongly preferred in Illinois.</li> </ul> <p><strong>Experience:</strong></p> <ul> <li>3+ years in a healthcare, care management, or clinical social work setting.</li> <li>Experience working in primary care or value-based care environments preferred.</li> <li>Proven ability to communicate effectively with physicians, practice staff, and patients.</li> <li>Demonstrated success in patient engagement, motivational interviewing, and addressing SDoH needs.</li> </ul> <p><strong>Skills &amp; Competencies:</strong></p> <ul> <li>Strong interpersonal and communication skills, with ability to build trust quickly.</li> <li>Comfort working in fast-paced, dynamic environments and adapting to evolving workflows.</li> <li>Proficiency with EHR systems (Epic, Athena, etc.) and care management platforms.</li> <li>Excellent organization and documentation habits.</li> <li>Empathetic, professional demeanor aligned with Phamily’s mission of patient-centered care.</li> </ul> <p><strong>Work Style &amp; Logistics</strong></p> <p>We are a fast-growing, early-stage company with a bold mission and significant work ahead; every employee at Jaan Health must embody growth company DNA.
